Chapter 8 – PERMIT APPLICATION – ADDITIONAL INFORMATION FOR NEW SOURCES AND NEW DISCHARGES
001 Application requirements for new sources and new discharges. New manufacturing, commercial, mining and silvicultural dischargers applying for NPDES permits (except for new discharges of facilities discharging non-process wastewater or new discharges of storm water associated with industrial activity which are subject to the requirements of Chapter 10 shall provide the following information to the Director, using the application forms 1 and 2C in Appendix C:
001.01 Engineering Report. Each applicant must report the existence of any technical evaluation concerning his wastewater treatment, along with the name and location of similar plants of which he has knowledge.
001.02 Other information. Any optional information the permittee wishes to have considered.
001.03 Certification. Signature of certifying official under Chapter 13 of this Title.
